The road to Mainpat winds gently upwards through forests and rolling hills until, almost without warning, the air begins to change. Cooler. Quieter. Lighter.
\nPeople often call Mainpat the \"Shimla of Chhattisgarh.\" I now understand why.
\nFor two days, this small plateau became home.
\nNot in a resort or a city hotel, but at an ex-Army officer's farmhouse — a modest homestay surrounded by pear trees, litchis, mangoes and countless other fruit trees. The owner had traded military life for farming, and somehow both identities were visible in the place. The discipline remained; only the landscape had changed.
\nAcross the road stood the Sangwari training centre. That was where the real journey began.
\nUnlike my previous visit to Jan Swasthya Sahyog, where I was witnessing a model that had evolved over decades, Sangwari felt different.
\nIt felt young. Still under construction. Still experimenting. Still learning. And strangely, that made it even more fascinating.
\nAshwini and Nisha became my guides over the next two days.
\nThere were no polished presentations waiting for us. Instead, there were conversations. Lots of them.
\nConversations about children.
\nNutrition. Apps. Government systems. Village workers. Migration. Trust.
\nAnd the thousand tiny decisions that determine whether healthcare actually reaches a family.
\nOne sentence stayed with me throughout the visit.
\n\n\n\"The real work is to find the child before the illness finds the child.\"
\n
Everything else seemed to flow from that idea.
\nSangwari's work begins long before a child reaches a doctor.
\nCommunity workers identify children showing early signs of severe malnutrition.
\nFamilies are counselled. Nutrition is monitored. Pregnant women are followed up. Vaccination status is checked. When required, children are referred to clinics.
\nIt sounds straightforward when written like this. It isn't.
\nBecause life refuses to stay inside neat program designs.
\nOne of the biggest challenges, I learned, wasn't nutrition. It was migration.
\nFamilies leave for brick kilns. Children disappear from records for weeks or months. Then they return.
\nHow do you run a nutrition program when your beneficiaries keep moving? How do you ensure continuity when the village itself keeps changing?
\nThe answer wasn't a sophisticated software system. It was relationships.
\nThe community workers knew families by name. They knew who had migrated. Who had returned. Whose child had been ill. Who needed another visit.
\nHealthcare, here, looked remarkably human.
\nWe later visited the Kuniya clinic. From outside, it looked unremarkable. Inside, it functioned like a beautifully choreographed orchestra.
\nPatients first stopped at registration. Then moved to the doctor. Some went for laboratory investigations. Others walked directly to the pharmacy. One small room handled dressing and minor procedures.
\nEverything was modest. Nothing looked expensive. Yet nothing appeared chaotic either.
\nIt reminded me that efficiency has surprisingly little to do with the size of a building. It has much more to do with clarity of purpose.
\nThe waiting area was full. Most patients belonged to tribal communities. Nobody had asked them to travel here. They came because they trusted the people inside.
\nTrust, I realised once again, is probably healthcare's most valuable infrastructure.
\nLater that day we visited one of Sangwari's nutrition homes. Calling it a \"centre\" almost feels misleading. It was simply a mud house. Except it wasn't.
\nInside lived something much bigger than the building itself. Two didis were caring for six young children.
\nThe walls had been painted using natural colours. The rooms were spotless. Everything had its place. The children were completely at ease. One corner stored food. Another held records.
\nWe tasted the sattu that forms an important part of the children's nutritional support.
\nSimple. Nutritious. Practical.
\nThe didis showed us their records. Every ration distributed. Every medicine given. Every child visited. Every follow-up done.
\nOnce again, I found myself wondering why we continue to believe that \"good systems\" only exist inside modern offices. Sometimes they exist inside mud houses.
\nPerhaps the most inspiring part of the visit happened the following morning.
\nAshwini and Nisha were conducting a training session for community health workers on the Avni application.
\nSome participants had started using the application only a month earlier. Some had barely begun.
\nI expected hesitation. Instead, I found curiosity.
\nQuestions kept coming.
\nEvery question represented someone trying to serve families better.
\nThe application itself is still evolving. A few issues surfaced that had escaped earlier testing. Nobody complained. Screenshots were taken. Feedback was noted. Workarounds were discussed.
\nEveryone seemed remarkably comfortable with one simple truth. The software was still work-in-progress.
\nAnd that was okay. Because improving it was everyone's responsibility. That mindset impressed me far more than the application itself.
\nOne discussion during the training stayed with me.
\nThe trainers repeatedly reminded everyone not to become obsessed with the app.
\nFirst understand the child.
\nThen understand the nutrition.
\nThen understand the program.
\nOnly then worry about the data.
\nThat felt like an important lesson for anyone working in digital health. Technology should never become the centre of the conversation. People should.
\nAs the visit progressed, another thought quietly emerged.
\nWe often judge organisations by outcomes. Number of beneficiaries. Projects completed. Awards received. Funding raised.
\nBut perhaps we should pay more attention to something else.
\nTheir systems. Not the software systems. Human systems.
\nHow people are trained.
\nHow knowledge is shared.
\nHow new workers are supported.
\nHow mistakes are discussed.
\nHow trust is built.
\nHow communities participate.
\nThese invisible systems determine whether visible outcomes last.
\nThroughout the visit, Sangwari reminded me of something I had been reflecting on since Anuppur.
\nHealthcare is rarely transformed by one big breakthrough. It changes because thousands of small systems begin working together.
\nA village worker identifies a child early. A nutrition didi prepares another meal. A nurse follows up. A doctor diagnoses on time. Someone updates a register. Someone fixes a bug in an app. Someone makes another home visit.
\nIndividually, these actions seem insignificant. Together, they become healthcare.
\nOn my final evening in Mainpat, I walked back towards the homestay. The orchards were quiet. The training centre across the road had fallen silent after a long day.
\nI found myself thinking about how different this visit had been from Anuppur. JSS showed me what decades of patient institution-building can create.
\nSangwari showed me something equally important. How those journeys begin.
\nWith rented buildings. Temporary clinics. Dedicated community workers. Evolving software application. Insightful feedback sessions.
\nAnd an unwavering belief that every small improvement matters.
\nPerhaps that is how meaningful institutions are built. Not through grand plans.
\nBut through thousands of ordinary days where people simply decide to return to work tomorrow and make one small thing better than it was yesterday.
